1. Permission for a return or an exchange can be obtained from a Preferred Medical Supplements LLC, Customer Service Representative, 9140 Corsea del Fontana Way, Naples, FL 34109. Only items approved in this manner will be accepted for credit. A Returned Merchandise Authorization form needs to be completed. The Preferred Medical Supplements LLC Representative is not authorized to remove goods from any place of business, regardless of the reason for the return.
    
2. Returns must be shipped prepaid to Preferred Medical Supplements LLC, 9140 Corsea del Fontana Way, Naples, FL 34109.
    
3. Products are accepted for return on the understanding that they are subject to our evaluation at time of receipt. We reserve the right to destroy products which are returned outside the above policy and which are unfit or unsafe for sale.
    
4. Preferred Medical Supplements LLC will consider all justifiable requests for return of merchandise for credit. The following however, are not considered returnable:
    
   • Merchandise expressly sold on a non-returnable basis or returned by someone other than the original purchaser.
   • Merchandise damaged by fire or improper storage, and merchandise that is involved in a fire, sacrifice or bankruptcy sale
